The above information is true and I authorize Twitchy Tails to contact those references listed above. I understand that the completion of this form is the first step in the adoption process and doesn't necessarily guarantee adoption. Twitchy Tails will have access to your home for cat drop delivery, which also serves as a home visit to ensure that a hazard-free home is provided. I agree to pay the non-refundable adoption donation, minimum $75 for nonmicrochipped cats, $100 for microchipped cats; $150 for kittens, $200 in cases of adopting kitten siblings. The adoption fee helps cover the cost of spay/neutering, testing for FIV/FeLV, vaccinations, deworming and treating for parasites.
